SELF ADMINISTRATION - ORAL

Indications for Prior Authorization:

  • Chronic management of hyperuricemia in patients with gout

Prior authorization criteria:

  • Patient has gout with symptomatic hyperuricemia, AND
  • Patient has tried and failed allopurinol, AND
  • Patient will not be concomitantly taking azathioprine or mercaptopurine

Dosing:

  • Initial: 40 mg once daily; for patients who do not achieve a serum uric acid less than 6 mg/dL after 2 weeks with 40 mg, Uloric 80 mg is recommended

Approval:

  • One year
